Pre-Enrollment Form

If you are interested in applying for Child Care for your child(ren), then please complete the Online Pre-Enrollment Form.

Child Health Report

Health Assessment Reports (physicals) and immunization records must be completed and returned within 30 days of enrollment. Infants and young toddlers must have a physical every six (6) months. Children ages 2-6 years old must have a physical every year to comply with our Licensing Regulations from the Department of Human Services.
